All of the clients details and information are bound by confidentiality law within Veterinary Surgeons Act 1966, RAMP code of conduct and HCPC code of conduct
The personal data we collect about you may include data relating to your name, address, contact details, your health/ medical history, your horses medical history, other professionals involved in you or your horses care.
This data is collected as a way of identifying individual clients and documenting treatment.
All data is stored securely on an electronic, password protected system and kept for up to 7 years, as required for insurance purposes. This information may be used by Award Equine Physio for communication with you regarding appointments, advice, offers and company updates.
The data collected will be used as record of you or your animals treatment and any changes that occur in you or your animals health.
You have the right to request access to your data, request corrections, if any errors in the data are identified, or deletion at any time.
Your horses information may be shared with your horses vet or any other professionals involved in their care for the purposes of keeping them informed of progress and changes with therapy. Your information will not be shared with anyone, unless previously agreed by yourself, which may include other health professionals involved within your care, unless threat to life or safeguarding issues are raised, which then would then initiate the physiotherapists code of conduct to report this to suitable services.
We may use video and/ or photo’s to document your horses progress, which will be stored securely on the note system. These may also be used on social media, please inform Award Equine Physio in writing if you wish photo’s not to be used on social media.
